In the Community

The majority of the care we provide is in community settings across Eastern Massachusetts and Southern New Hampshire — beyond our academic medical centers and teaching hospitals. We want to extend and strengthen our ambulatory and community care network to provide patients with access to the full capabilities of an academic system close to where they live and work. As part of this commitment, we are establishing a bold goal for 70% of the care we provide to be delivered in the community by 2030.

Through investments in behavioral health care, health disparities and social determinants of health, we will have meaningful impacts in our community.

By forming new partnerships and expanding services outside our existing geographic footprint, we will bring our system of care to new communities.

In the Home

We are redesigning home care to enable a much broader set of solutions — spanning virtual care and remote monitoring to hospital-at-home services — to manage complex diseases in the home with new capabilities.

At Your Fingertips

Our ongoing digital transformation will help reshape the patient journey and make it easy for patients to find us, book an appointment and connect with their care teams — all online. We will also build a unified system telehealth platform that supports access to high-quality care. Through various digital patient engagement tools, patients will have a range of options for connecting with their care teams.
