At Beth Israel Lahey Health Primary Care – Maple Street, we offer comprehensive primary care services across your entire lifecycle. Our team of experienced providers are certified by the American Board of Internal Medicine and the America Board of Family Medicine and take a team-based approach to your care.
Patient Information
Our Services
You should see your primary care provider once a year for your annual check-up. You may need additional visits if medical issues arise. We provide the following services:
On-Site Services
- Same-day sick appointments
- Blood draw
- Access to services at Lahey Outpatient Center, Danvers, which is an outpatient medical and day surgical center offering a full range of services, including breast health, cardiovascular disease management, endocrinology, infectious disease, lab, MRI/Radiology, OB/GYN, orthopedics, outpatient surgery, pain management, physical and occupational therapy, podiatry, urology, and much more.
Preventive Care
- Adult physicals, including work physicals
- Family medicine, including immunizations
- Day care and camp physicals
- School and sports evaluations
Management of Chronic Medical Conditions

Specialty Care & Referrals
If you feel you need to see a specialist and have not discussed the referral with your primary care provider (PCP), please first call your PCP, as they may need to see you prior to referring you to the specialist.
When needed, your primary care provider works with other subspecialty providers at Beth Israel Lahey Health to ensure you have the care you need to feel your very best.
