Requesting Medical Records

There are times when it is necessary to request your medical records.

The following individuals can request patient records:

• Patients requesting their own records
• Parents of minor patients requesting records
• Caregivers acting on behalf of a patient (i.e., Power of Attorney)

Electronic Request for Records

There are two ways to request a copy of your medical information that may be used to make decisions about your care. The easiest way is to log into your patient portal. The portal allows you to inspect your records and doctor's notes. You can also request a physical copy of your records.

Paper Requests for Records

To have a copy of your medical records sent to another medical facility or to obtain a copy for personal reasons, please reach out to the Beth Israel Lahey Health entity where you receive your care.
